Items and Claims
Procedure For Damaged Items:
- Delivery Claim Procedures For Owning Libraries
Only.
- Please submit the claim form immediately
upon noticing damage, clearly describing
the nature of the claim. Click here for
claim form.
- Damaged items must be sent to the SEMLS
office after your claim is submitted online.
Procedure For Items Missing In Transit:
STEP 1: (For any item that
has been in transit for 2 weeks)
Double check your shelves or other areas
(repair and/or processing.)
- Ask requesting library to double-check
their facility for item.
- Complete the online notification form.
- Allow two weeks after submission of this
form to see if item returns to your collection
before going on to step 2.
Procedure For Claims
STEP 2:
- Allow 2 weeks after submission of this
missing item notification form to see if
the item returns to your collection.
- If it still appears on your In Transit
report on the 4th week, a Claim Form, available
on our web page should then be submitted.
- If available, please submit a copy of
your Missing In Transit Items report generated
by your local automated system.
SEMLS will not be responsible for
items missing in transit for more than six
months.
